This form is for a replacement certificate only. You cannot request a baptismal certificate unless the person you are requesting for has already been baptized. If you are looking to get your child baptized or be baptized yourself, please email maryruthb@johnthebaptist.org

Address to mail baptismal certificate to if you are unable to pick it up in person from St. John's. Please note that we will only send it to either your local parish that needs it, or to an address that matches that on the photo ID uploaded.

Last things to note

Certificates for adults will only be released to the adult on the certificate or the church requesting the certificate.
Please be advised, fraudulent documents will be reported to the appropriate authorities.
(An individual must be baptized at St. John’s for a certificate to be prepared. We do not prepare baptism certificates for other parishes)
