This form is for a replacement certificate only. You cannot request a communion certificate unless the person you are requesting for has already received Holy Communion. Last things to note

Certificates for adults will only be released to the adult on the certificate or the church requesting the certificate.
Please be advised, fraudulent documents will be reported to the appropriate authorities.
(An individual must have received First Holy Communion at St. John’s for a certificate to be prepared. We do not prepare First Holy Communion certificates for other parishes.
