This form is for a replacement certificate only. You cannot request a marriage certificate unless the person you are requesting for has already been married at St. John's. If you are looking to receive the sacrament of marriage yourself, please email maryruthb@johnthebaptist.org

Address to mail marriage certificate to if you are unable to pick it up in person from St. John's. Please note that we will only send it to either your local parish that needs it, or to an address that matches that on the photo ID uploaded.

Last Things to Note

Certificates for adults will only be released to the adult on the certificate or the church requesting the certificate.
Please be advised, fraudulent documents will be reported to the appropriate authorities.
Individuals must have been married at St. John’s for a certificate to be prepared. We do not prepare marriage certificates for other parishes)
